on high full minute billing increments (the time you pay for but don't
use on the last minute of your call)

on high minimum connection times (1-3 minutes)

on lost or misplaced cards

on connect fees each time you make a
call

on high pay phone connect fees
-
on short card expiration times, e.g., 60-90 days, some even 30 days
-
on monthly (sometimes weekly) "maintenance" fees
-
on a combination of these items
